Abstract:
The core damage accident in the Three Mile Island Unit 2 reactor included several coincidences related to:

  • (1) the start-up instrumentation that, strictly by chance, happened to still be in-place, and
  • (2) sequence related events which occurred during the accident, at times that resulted in essential Accident Management (AM) insights.

These insights will be discussed along with along with how such insights are used in formulating the Severe Accident Management Guidelines (SAMGs) for light water moderated reactors.
