Important health coverage tax documents (1095-c)
Form 1095-C is a tax form required by the Affordable Care Act (ACA) that indicates whether Argonne offered you the opportunity to enroll in a medical insurance plan and whether you accepted the offer for each calendar month of the year. This form will only be available upon request, or through electronic delivery through your MyTaxForm account if you consented to electronic delivery. If you did not consent to receive your tax form electronically through MyTaxForm, and you would like a copy, you may request a copy of your 1095-C by:
- Emailing your request to benefits@anl.gov (include name and badge number)
